For Damage to Your Vehicle:

• Claim form
• Police report
• Registration card (copy)
• Road tax disc (copy)
• Driver's license and IC (copy)
• Vehicle owner's driving license and IC (copy)
• Workshop estimate for repairs
• Hire purchase agreement where applicable (copy)

For theft of vehicle:

• Claim form
• Police report
• Registration card(copy)
• Policy and certificate of insurance (copy)
• Letter of release from the Finance Company, where applicable
• JPJ K3 duly signed (2 sets)
• Full set of keys to vehicle
• Insured's IC (copy) for indiviuals, sole-proprietorship and partnership OR Certified copy of Memorandum & Article of Association (for Limited Company)

For windscreen claim:

• Claim form
• Photographs of the damaged windscreen and vehicle
• Cash bill / Replacement invoice
• Registration card (copy)

For claims received from third parties:

• Notice of accident
• Police report
• Police sketch plan with key
• Driver's license and  (copy)
• Registration card and Road tax (copy)
• Correspondence received from Third Party Claimant / Their solicitors
